1. The complete list of what we collect
If you use the waitlist form, we store the email address you entered, your optional menu answer, and a timestamp. If you use the clinician form, we store your email, your two optional menu answers, and a timestamp. That is the entire list. If you never use a form, this site stores nothing about you.
2. What this preview does not do
No analytics scripts run on this preview. No advertising pixels, no third-party trackers, no cookies set by us, and no requests to third-party servers from your browser. Fonts and graphics are served first-party. We do not collect health information anywhere on this site, and neither form asks for any.
3. How form data is used
Waitlist entries: to email you when programs open, occasionally before that with guides, and to set onboarding order at launch. Clinician entries: to reply to you about the leadership search. We do not sell or share this data with anyone, and we will not, full stop.
4. Where it lives
Entries are stored in a private server-side file on the preview infrastructure. Before launch, entries will move into a production system with access controls, or be deleted.
5. Deletion, from day one
Ask and your entry is deleted. Reply to any email we send, and the launch site will also carry a direct contact address for privacy requests.
6. Health information at launch
When care programs open, health information will be collected only inside the care flow, under a HIPAA-compliant arrangement described in the final version of this notice and in the telehealth consent. The preview site you are reading now collects none.
7. Age
This site is for adults. It is not directed at anyone under 18, and we do not knowingly keep entries from minors.
8. Changes
This draft will be replaced by a counsel-checked version before launch, posted at this address with a date.
